Psychosomatics of a herniated disc (spinal hernia)

A herniated disc is when the soft cushion between the vertebrae bulges out and can press on a nerve. Your back or neck hurts, and you may feel numbness or pain in a leg or arm. Many discs cause no pain at all, and painful ones usually settle within weeks or months with movement and painkillers. Numbness around the groin and being unable to pass urine is an emergency. The Ladder method adds questions about the load you carry.

When to see a doctor

Call an ambulance (103) if back pain comes with numbness around the buttocks and genitals, if you cannot pass urine or hold urine or stools, if feeling is lost in one or both legs, or if the pain started after a serious injury such as a car accident. See a doctor urgently if the pain comes with a high temperature, you lose weight for no reason, or the pain is worse at night.

Start with a general practitioner or a neurologist: they will examine you, check feeling and strength, and decide whether an MRI and physiotherapy are needed. The NHS advises seeing a doctor if painkillers do not help or the pain does not go away after a few weeks.

In the Ladder method

In the Ladder method a herniated disc is not explained by "you take on too much", and you are not to blame for the disc bulging. A disc has clear causes: load, age, heavy lifting, heredity. But back pain gets worse with tension and lack of sleep, so questions to yourself are appropriate here.

In the method, an aching back fits the image of a load: you carry other people's tasks, responsibility and expectations. It is an image, not the cause of the hernia, and it does not replace an examination and treatment.

On the ladder this shows up as the rungs "if not me, then no one" and "asking for help is not allowed". You can work through them while the doctor chooses treatment.

These are questions for reflection, not an explanation of what causes the condition and not a treatment.

Quick check: what may be behind it

  1. Which load do you carry alone that you could share?

  2. Who could take over part of your tasks today?

  3. When did you last take a break not because of pain, but because you wanted to rest?

How it shows up

Book a doctor’s appointment if you notice:

  • Pain in the lower back or neck
  • Pain spreading to the buttock, leg or arm
  • Numbness and tingling in arms or legs
  • Weakness in a muscle of the arm or leg
  • Hard to bend or straighten up
  • Pain worse with coughing, sneezing, long sitting

What medicine knows

A herniated (slipped) disc is when the soft cushion between the vertebrae bulges outward. It becomes painful if it presses on a nerve. Signs: pain in the lower back or neck, numbness and tingling in arms or legs, muscle weakness, and pain in the buttock and leg when the sciatic nerve is pinched (NHS).

Not every disc causes symptoms: many people never find out they have one. Back pain is rarely caused by something serious. According to the NHS, it usually improves slowly on its own with rest, gentle activity and painkillers.

What helps: staying active. If the pain is very strong, rest for the first few days, then gradually return to walking, swimming or yoga, always without pain. Take anti-inflammatory painkillers as the leaflet says and after advice from a pharmacist or doctor.

If the pain does not go away within a few weeks, a doctor may order an MRI or physiotherapy, and in rare cases surgery. A doctor diagnoses from your symptoms, not from a scan found online: discs show up on MRI even in people with no pain.

Frequently asked questions

Can a herniated disc come from nerves?

Nerves do not create a hernia: it is a change in the disc from load, age and other physical causes. But tension, stress and lack of sleep can make back pain worse. Treatment is a doctor's job.

What does a herniated disc mean in psychosomatics?

A hernia has no proven "meaning". In the Ladder method back pain becomes a reason to ask yourself what load you carry and whether you can share it. It is a question to yourself, not a diagnosis.

When does back pain need an ambulance?

If numbness appears around the buttocks and genitals, you cannot pass urine or hold urine and stools, or you lose feeling in your legs. Call an ambulance (103).
