Psychosomatics of irregular periods

Periods are irregular when the length of your cycle keeps changing. A cycle usually lasts 21 to 35 days, and small shifts are normal. Puberty, the start of menopause, pregnancy, contraception, sudden weight change, stress, hard training, PCOS and thyroid problems can all throw it off. A gynaecologist looks for the cause. The Ladder method adds questions about whose rhythm you live by, and it does not replace a doctor.

When to see a doctor

See a doctor urgently if the bleeding is very heavy and you soak a pad in an hour or less, you feel dizzy, you faint or you have bad tummy pain. Go to emergency care straight away if you could be pregnant and have severe pain low in your tummy or bleeding.

A gynaecologist looks at irregular periods. The doctor asks about your cycle length, weight, exercise and medicines, may do a pregnancy test, blood tests for hormones and thyroid, and a pelvic ultrasound. Treatment is chosen to fit the cause.

In the Ladder method

In the Ladder method an irregular cycle is not explained as "you do not accept yourself as a woman". You are not to blame. It is worth finding the medical cause with a gynaecologist first. Questions to yourself belong next to the check-up, not instead of it.

In the method the cycle fits the image of "your own rhythm": whether you live at your own pace or fit yourself to other people's deadlines, work and expectations, and whether your week has any rest. Stress really can throw the cycle off. It is an image, not the cause, and it does not replace treatment.

On the ladder this shows up as the steps "I have to get everything done" and "rest can wait". You can work through them at your own pace alongside the check-up and your cycle tracker.

These are questions for reflection, not an explanation of what causes the condition and not a treatment.

Quick check: what may be behind it

  1. Whose rhythm do you fit your days to?

  2. When did you last rest without feeling guilty?

  3. What changed in your life when your cycle went off track?

How it shows up

Book a doctor’s appointment if you notice:

  • Your cycle is sometimes shorter than 21 days and sometimes longer than 35
  • More than 20 days between your shortest and longest cycle
  • Periods that last more than 7 days
  • Bleeding between periods
  • Periods that stop for months
  • Trouble getting pregnant

What medicine knows

The NHS says a cycle usually lasts 21 to 35 days. Periods are called irregular when the length of the cycle keeps changing and they come earlier or later.

Periods are often irregular in the first years after they start and before menopause, usually between 45 and 55. They can also change in early pregnancy, with hormonal contraception and after stopping it.

Sudden weight loss or gain, severe stress and very hard training can throw the cycle off. Common medical causes are polycystic ovary syndrome (PCOS) and thyroid problems.

Treatment depends on the cause. Sometimes it is enough to track your cycle in an app and watch, sometimes you need tests and hormonal medicines. If your cycle has changed and you do not know why, it is worth seeing a gynaecologist.

Frequently asked questions

What cycle is normal?

A cycle usually lasts 21 to 35 days, and a small difference from month to month is normal. It is called irregular when the length keeps changing (NHS).

What do irregular periods mean in psychosomatics?

There is no proven "meaning". In the Ladder method an irregular cycle becomes a reason to ask yourself whether you live by your own rhythm and whether your days have rest. It is a question to yourself, not a diagnosis.

Can stress throw my cycle off?

Yes, the NHS lists stress among the causes of irregular periods, along with sudden weight change and hard training. But there are medical causes too, such as PCOS or thyroid problems, so it is worth seeing a gynaecologist.
